JOB DESCRIPTION

• Recommend and connect patients with various programs and services
• Plans and manages patient care according to each patient's needs.
• Evaluates the effectiveness of the Nursing care.
• Foster continuity and quality of care for the well-being of patients.
• Assist in developing individualized treatment plans.
• Comply with evaluation, treatment, and documentation guidelines.
• Participate in clinical team conferences and cohort new grad support meetings. Requirements:
  • Associate Degree -ASN or Diploma in Nursing
  • BLS, ACLS certification
  • 1-2 years recent Med/Surg experience License/Certification:
    • BLS(AHA) and ACLS and state license.
    • One year of RN ICU/CCU experience preferred. Skills:
      • Computer charting experience EPIC.
