(LTC) Physical Therapist for a travel job in Wills Point, Texas.

(Glassdoor posting, 2025). Position SummaryThe Physical Therapist
will lead the evaluation and treatment of patients with a wide
range of diagnoses including orthopedics, neurology,
cardiopulmonary, and geriatrics. This individual will develop and
implement individualized care plans, collaborate with
interdisciplinary team members, educate patients and families, and
support smooth transitions of care. Key Responsibilities - Conduct
comprehensive PATIENT evaluations and functional assessments to
identify impairments, activity limitations and participation
restrictions. - Develop and implement individualized PHYSICAL
THERAPY care plans utilizing evidence-based interventions,
modalities, manual techniques, therapeutic exercise, gait training
and functional training. - Monitor and document patient progress,
modify interventions as needed, and discharge patients effectively
to maximize outcomes. - Educate patients, families and caregivers
in home exercise programs, adaptive techniques and safe functional
transfers. - Collaborate with interdisciplinary team members
including nursing, speech, occupational therapy, dietetics and case
management to ensure coordinated care delivery. - Participate in
program development efforts including specialty offerings (e.g.,
stroke rehabilitation, dysphagia management, cardiopulmonary
recovery) (Glassdoor posting, 2025). - Uphold regulatory, clinical
and documentation standards for skilled rehabilitation and
post-acute care settings. Qualifications - Bachelor’s or Master’s
degree in Physical Therapy from an accredited program. - Active
state license as a Physical Therapist in good standing. - Strong
knowledge of anatomy, physiology and rehabilitation principles. -
Excellent communication, interpersonal and organizational skills. -
Preferred: prior experience in a skilled nursing, long-term care or
post-acute setting. New graduates may be considered if they
